Health Information Manager

Department: Compliance
Location: Clearfield, PA

Job Summary:

As a Health Information Manager, you will play a key role in keeping participant health records accurate, secure, and accessible. You’ll support clinical and administrative teams by ensuring documentation is complete, compliant, and aligned with PACE program standards. Your work helps every department deliver safe, coordinated, high-quality care.

This role is ideal for someone who is organized, analytical, and committed to protecting the integrity of participant information. You’ll work in a calm, team-centered environment where accuracy, collaboration, and continuous improvement are valued.

Schedule:

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties and Responsibilities:

Health Information Management & Compliance

  • Develop, implement, and maintain policies and procedures for health information management in alignment with PACE regulations and HIPAA standards.
  • Ensure confidentiality, security, and integrity of all participant health records (electronic and paper).
  • Monitor compliance with CMS, state, and organizational requirements for documentation and reporting.
  • Conduct regular audits of medical records for accuracy, completeness, and timeliness.

Electronic Health Record (EHR) Oversight

  • Manage and optimize EHR workflows to support interdisciplinary care planning and regulatory reporting.
  • Train staff on proper documentation practices and system use.
  • Troubleshoot EHR issues and coordinate with IT for system enhancements.

Data Management & Reporting

  • Oversee coding and classification of diagnoses and procedures for accurate billing and reporting.
  • Generate and analyze reports for quality improvement, risk management, and regulatory submissions.
  • Support data collection for PACE quality measures, UDS reporting, and other performance metrics.

Interdisciplinary Collaboration

  • Work closely with providers, nurses, care coordinators, social workers and all PACE staff to ensure timely documentation of assessments, care plans, and transitions of care etc.
  • Serve as a resource for clinical teams regarding documentation standards and regulatory requirements.

Risk Management & Privacy

  • Implement and monitor disaster recovery plans for health information systems.
  • Investigate and resolve data integrity issues and privacy breaches in accordance with policy.

Education and Experience:

  • Bachelor’s degree in Health Information Management, Healthcare Administration, Nursing, or a related field
  • RHIA or RHIT certification preferred
  • 3–5 years of experience in health information management, ideally in PACE or long-term care
  • Knowledge of HIPAA, CMS, and state regulations
  • Proficiency with EHR systems and medical coding (ICD-10, CPT)
  • Strong analytical and organizational skills
  • Ability to manage multiple priorities in a fast-paced environment

Preferred Qualifications

  • Experience working within PACE program workflows
  • Familiarity with quality reporting tools and risk-based care models

Requirements:

  • Valid state driver’s license with a clean driving record
  • Successful completion of background check, physical, drug screening, and TB screening

Physical Requirements:

  • Able to move intermittently throughout the workday
  • Able to sit, stand, bend, lift, and move during working hours
  • Able to drive a motor vehicle as needed
  • Occasional travel for meetings, training, and continuing education
